J Clin Oncol 2017;36:383-90. La inmunoterapia es una modalidad de tratamiento sistémico clave para el melanoma metastásico, tanto BRAF wild type como BRAF mutado. Los resultados con la aplicación de la moderna inmunoterapia en melanoma han conducido a la aprobación de los anticuerpos monoclonales anti-CTLA-4 (ipilimumab) y anti-PD-1 (nivolumab y pembrolizumab). Con su aplicación, la supervivencia de los pacientes con melanoma avanzado ha aumentado del 20-25 % al 55-60 % a los 2 años. Los anticuerpos contra la proteína PD-1 (nivolumab y pembrolizumab) han demostrado aumentar la supervivencia, el tiempo hasta la progresión y la tasa de respuestas frente a quimioterapia y frente a ipilimumab (anticuerpo anti-CTLA-4), y se consideran un tratamiento estándar en primera línea en pacientes con melanoma BRAF wild type. La mayor eficacia de la combinación anti-CTLA-4 + anti-PD-1 viene acompañada de una mayor toxicidad que, si bien es manejable, hace que el tratamiento deba ser administrado en centros y por profesionales con experiencia en el manejo de efectos adversos relacionados con esta autoinmunidad yatrogénica.
